Use this form to let the Board know about a covenant violation, maintenance concern, or other neighborhood issue.

A note on anonymity and police involvement: Reports that do not require law enforcement involvement may be submitted anonymously. However, if the nature of the report is such that police involvement becomes necessary, Oklahoma City Police Department policy generally requires a named, identifiable complainant in order to file and process a police report. In that circumstance, the Board will be unable to forward your report to law enforcement unless your name and contact information are included.
